All contractors and any of their employees and/or sub-contractors are required to be provided with a copy of the the Contractor & Tradespersons OHS Hand Book and they must then read and sign off on the Contractor Agreement Form (located on back page of Hand Book) The contractor may require more than one copy if they have employees or are going to engage sub-contractors.

Contractor Agreement Form:

Before the contractor, their employees and/or any sub-contractors commence works, they are also required to complete a Contractor Agreement Form. This document is to be kept by the congregation or Synod department for 7 years.

Job Safety Analysis:

A contractor or tradesperson is required to submit to the congregation or Synod department that is engaging the contractor a Job Safety Analysis (JSA) prior to commencement of works. Any sub-contractors to a contractor are also required to supply Job Safety Analysis for the work to be undertaken.
